Coverage ends at midnight on the last day of employment. You may apply to convert this coverage to individual policies for yourself within 60 days of your coverage termination date. You will be responsible for paying the entire premium to New York Life to keep this coverage in force. Supplemental Life Insurance (Employee and Dependents)

Coverage ends at midnight on the last day of employment. Premium deductions will apply through the pay period that includes the employee’s last day of employment. You may apply to convert this coverage to individual policies for yourself and eligible dependents within 62 days of your coverage termination date. You will be responsible for paying the entire premium to New York Life to keep this coverage in force.
